First, it was the shocking San Quentin slot, but in this horror-themed release you’ll find yourself in a dingy mental institution. Be ready to face some brutal bonus features where Lobotomy is still a thing, and dead multipliers can appear from the darkest corners.
As well as the game’s gory features, the symbols aren’t any friendlier either. The low paying icons come as the brain, lungs and other human organs, while the high paying symbols are the disturbed patients.

Mental Bonus Features
The team behind Nolimit City did not spare any time nor energy when crafting the bonus slot features appearing in their latest panic-filled creation. So, be prepared; this slot is fast-paced and positively complex.
Luckily, most of the initial features, containing additional sub-features, can be triggered from the base game, including the free spins bonus rounds. Wilds
Skulls of the deceased patients act as wild symbols that can improve and complete a winning combination. They substitute for all other symbols, except for scorpions, spiders and dead patients. Additional xNudge wilds can also appear in later stages of the game.
Fire Frames
This is a random event during which any of the grid’s 13 cells can catch fire, and any symbols that land within the burning positions split into two. 4-6 fire frames will activate the Enhancer Cells feature.
Dead Patient
Triggered with 2 dead patient symbols, the Dead Patient feature reveals one premium dead patient with a x10, x15, x30, x100, x200, x500, x1,000 and a truly mad x9,999 multiplier.
Enhancer Cells
As seen in the developer’s previous releases, Enhancer Cells can be found at the bottom reels 2 and 4. When triggered, they reveal the following side effects:
- 1 of the 5 patient symbols.
- A standard dead patient symbol.
- A regular wild symbol.
- xNudge wild which always nudges into full view and increases the multiplier 1x for each nudge it takes.
- xWays. This will expand the reels and reveal 3 identical symbols except a scorpion, spider and dead patient.
- xSplit. Splits any symbols into two before splitting itself into 2 wilds.
Mental Transform
Initiated with a spider symbol on reels 2 or 4, the feature turns the triggering spider into either xWays, xSplit, dead patient, wild or regular patient symbols.
Free Spins
As mental as this slot is, it comes with 3 different free spins bonus rounds, each with its own special boosters.
Landing 3 scorpion symbols on reels 1, 3 and 5 in the base game will activate the Autopsy Freespins bonus round with 8 free spins. If any of the scorpions land in the fire frame, you’ll pick up an extra 1 free spin. During this round, you can have up to 5 sticky fire frames added.
A combination of 3 scorpions and 1 spider symbol leads you to the grim Lobotomy Freespins round with 9 initial free spins and game boosters from Autopsy Freespins. Each time you receive a boost, the multiplier count decreases.
Last but not least is the Mental Freespins bonus round featuring 10 free spins and game enhancers from the previous Lobotomy round. This round activates with a mix of 3 scorpion symbols and 2 spiders, both of which are sticky for triggering the Mental Transformation feature. Unlike Lobotomy free spins, Dead Multipliers here are persistent for as long as new patient symbols are revealed.
